Appointment/Service Terms and Conditions Page

"I", "Me," "We," "Us" refers to the Lead Esthetician, Shannon, Sweet Life Skin Care and Beauty Treatments, LLC, and skincaresweets.com. "You" and "the client" refers to all who continue into creating an appointment with Sweet Life Skin Care and Beauty Treatments, LLC or into the treatment room for a skincare, wax, lash or brow service provided by the Lead Esthetician, Shannon. Appointments for skincare, wax treatments to the face and body, facial threading, lash and brow treatments (referred to as “service” or  “services'') are available for existing and new client(s) (“you”),  to create at https://www.fresha.com/book-now/sweet-life-skincare-and-beauty-treatments-llc-w1fdpc2g/services?lid=1213111&eid=2835140&pId=1151046, www.skincaresweets.com. These links may also be accessed on Instagram at www.instagram.com/sweetlifeskinandbeauty and www.instagram.com/lys.m714.

You, the client, observe and agree to pay the pre-determined and required deposit of 30% of the selected service(s) or 100% of the service due at the time of booking your appointment, and agree that all appointments are subject to cancellation by the esthetician until the appointment is accepted by the professional.  

Your ‘Name, Email Address and Phone number’ are required to set your appointment. Your information such as the above mentioned and any credit or debit card information (i.e. credit and debit card numbers, zip code, billing address) will be collected and shared with 3rd Party App Developers at Shopify, Google, Fresha, Picktime, Instagram, and PayPal, as well as private and national banking institutions.  Within 24 hours of receiving your reservation - the local esthetician will be contacting you with the information you provided while creating your appointment, to confirm your appointment details. Any appointments lacking contact information will be canceled along with the deposit canceled.

 Contact me, Shannon, directly if you need assistance in setting your appointment or correcting any errors made to your booking. I will be contacting you based on your appointment information to confirm your appointment! 

A cancellation fee of $20 will be incurred if you cancel within 72 hours of your appointment.  You agree that all no shows will incur a fee, in addition to your initial deposit. 

Your deposit will not be refunded due to any rescheduling of your appointment, and any changes to the service or services may also impact the deposit due. In case of your deposit reducing, the balance will be credited to your store account for future use on select services from Sweet Life Skin Care the 48 hour confirmation window, your deposit will be fully refunded and visible in the account from which you paid within 5-10 business days.  By continuing with this agreement, you agree that after 24 hours of setting your appointment with us, you forgo any refunds by canceling your appointment with Sweet Life Skin Care and Beauty Treatments, LLC after confirmation. Failure to confirm or cancel your appointment within the 24 hour confirmation window incurs in a non-refundable deposit. In the event of rescheduling, you may reschedule an appointment for up to one (1) month after notifying your esthetician within the confirmation window before a new deposit is due, to create a new appointment. Please arrive to your appointment with Sweet Life Skin Care and Beauty Treatments, LLC 10-15 minutes early to complete new client registration and paperwork. Loose fitting attire is suggested, use the restroom before entering the treatment room, and be prepared to restore and rejuvenate your skin. Your phone must be silenced and put away during your skincare, lash or brow treatment to avoid any injury. 

All clients/guests are required to be 18 years or older to receive treatment and set an appointment - unless accompanied by an authorizing and supervising parent/guardian. The parent/guardian is required to sign treatment release forms to ensure accurate responses to the health history questionnaires, to authorize treatment plans, and to review and supervise all payment plans and agreements. 

Remember to be courteous at all times. The company and I hold the right to refuse service for any reason. All clients and those attending the brick and mortar or online establishment of Sweet Life Skin Care and Beauty Treatments, LLC or skincaresweets.com, or seeking service from this establishment are considered to be adults or under adult supervision. You are responsible for your behavior and as such, will uphold high standards of kindness and consideration for the business and me, as your esthetician, and all other clients/guests. All parents/guardians are responsible for their children at this establishment and agree to close supervision. Ill mannered children will not be tolerated, and will result in cancellation of the service if behavior becomes a problem. This applies to the parent/guardian as well.

By continuing with this agreement and finalizing your appointment booking, you agree that though all services and treatments are and will be provided with close observance to the safety of you (the client) and your trusted professional - you waive all rights to imposing liability to the business corporation, its counterparts and its proprietor - Shannon Mitchell - due to any complications with the service or treatment in the event you do not provide all necessary insight into your existing conditions. Sweet Life Skin Care and Beauty Treatments LLC and its employees will not provide a diagnosis for any conditions or disorders you may have. You must seek these from a licensed medical professional. You agree that you are responsible for accurately completing all health questionnaires, release and consent forms prior to your appointment. Quality of service is very important to us as a company  as it is to you, our valued customer and client.

 The company aims to provide a quality experience and will do its best to respond to customer feedback to improve all customer experiences; however, misleading, slanderous claims or misrepresentation and defamation of the establishment and/or products or services given will not be tolerated and will be subject to legal action being taken according to the jurisdiction of the geographical location of where the claim took place. By continuing onto creating your appointment, you agree that this relationship entails that you will report all feedback, changes, observations, and experiences due to the treatment of the treated area at Sweet Life Skin Care and Beauty Treatments, LLC to the licensed esthetician for review by the Lead Esthetician at Sweet Life Skin Care and Beauty Treatments, LLC to ensure that the proper procedures for safety and treatments are being provided to you as our client and to guide you in case of complications, referring you to your physician or the E.R. in case of emergency.

By continuing to book your appointment, you agree that your review after receiving the services provided at Sweet Life Skin Care and Beauty, LLC on public forums, will not be defamatory, damaging or malicious regarding the establishment, its services, its technicians nor its counterparts. At Sweet Life Skin Care and Beauty Treatments, LLC you are a valued part of our business and our aim is to provide a top quality experience for your skincare, wax, brow or lash treatment. We hope to foster relationships of community and growth and are dedicated to making sure that you feel refreshed and rejuvenated after every appointment, reveling in your decision to visit our studio.  If you have any questions or concerns, always feel welcomed to discuss these with the leading esthetician at Sweet Life Skin Care and Beauty Treatments, LLC. We respect your right to complain and hope to alleviate your concerns.

Sweet Life Skin Care and Beauty Treatments, LLC. reserves the right to change, add, replace or update these terms and conditions without prior notice and regardless of any prior transactions regarding the current terms and conditions. All bookings approved according to the current terms and conditions prior to any updates to the agreement (“terms and conditions”) will always be subject to the most up to date terms and conditions.

Contact your esthetician at Sweet Life Skincare and Beauty Treatments directly to update any errors made to your booking or so that I may assist you with potential difficulties in creating your appointment.

Failure to confirm/cancel your appointment within 48 hours of your scheduled appointment results in your deposit being non-refundable; calling to cancel your appointment within 48 hours of your scheduled appointment, without you confirming your appointment will not incur a fee. All appointments not confirmed within 24 hours will not be honored. In case you cancel your appointment within 24 hours of making your reservation, 100% of your deposit will be returned to you.   You agree that, upon confirming your appointment, in the case you make any cancellations during the timeframe between scheduling your appointment and 24 hours before arriving to your appointment at Sweet Life Skin Care and Beauty Treatments, LLC - you will not be refunded.  I, Shannon -  the lead esthetician, will contact you within 48 hours of your appointment to confirm your attendance.
